Background CD4 (cluster of differentiation 4) is a glycoprotein that serves as a co-receptor for the T-cell receptor (TCR). CD4 is found on the surface of immune cells such as helper T cells, monocytes, macrophages, and dendritic cells. CD4 is an approximately 55 kDa type I transmembrane glycoprotein. It is a standard phenotype marker for the identification of T cell populations. CD4 binds directly to MHC class II molecules on antigen presenting cells. This interaction contributes to the formation of the immunological synapse which is focused around the TCR-MHC class II-antigenic peptide interaction. CD4 also functions as a chemotactic receptor for IL-16 and as a co-receptor for the gp120 surface glycoprotein of HIV-1. Measuring the number of T cells expressing CD4 can be used to assess the immune system of a HIV patient. CD4 continues to be expressed in most neoplasms derived from T helper cells. It is possible to use CD4 immunohistochemistry on tissue biopsy samples to identify most forms of peripheral T cell lymphoma and related malignant conditions. The antigen has also been associated with a number of autoimmune diseases such as vitiligo and type I diabetes mellitus.
