How to Request a Promotional Item

1. Place an order and write down the sales order number (the number must start with SR).
2. Return to this page, enter your sales order number and fill out the rest of the form below.
3. You will receive your gift 4-6 weeks after the end of the promotional period when your entire order is shipped, completed and invoiced.
4. If you decide to cancel or adjust your order after the confirmation is made, the amount will be adjusted accordingly to reflect the change.
5. The address you enter below will be the shipping address for your promotional item.
